My phone did the exact same thing yesterday while my Bluetooth braille display was connected. Mine was not a volume problem as this was the first thing that I tried to recover voiceover. I didn’t get VoiceOver back until I turned off the braille display so I think it was the Bluetooth hijacking the audio as someone in this thread proposed. It’s a weird and interesting problem and can be a little nerve-racking when it happens.
