I tested it, and it's not a Voiceover problem. I believe it occurs when I am in a media app. A message (or any other notification) will show up in the notification center, but not as a banner. The notification settings are normal. I assume this is the bug described, but I didn't notice it until recently, and I've had iOS 14 for awhile.
