Check your settings, Sound, change with buttons. If that is not off, that's your problem. I keep that setting off, so the volume buttons don't mess with VoiceOver. I just connected a Bluetooth speaker, played a BARD book, playing with the volume buttons all over the place, and VoiceOver never got out of line. I.E. Its volume stayed normal.

Go into bard and start playing a book Use the volume up and down button to set the volume of the speaker you're playing through. While in bard just use voiceover normally. You'll find that there are two volumes with voiceover. One of them is the volume that you set in the rotor and the other one is actually unknown to me at this point that one is very loud and uncontrollable.
