I wonder if this might help. When it happens, turn the volume on your phone all 
the way down, until you don't hear anything. Now, turn the volume on your 
speaker all the way down, hit it three or four times to make sure it is all the 
way down. Now, start turning up the volume on your phone until the
volume is what you want. Or, sometimes, you have to turn up the volume on your 
speaker, rather than the phone. Sometimes one, sometimes the other. Lay around 
with it to see if you can fix it.

Check your settings, Sound, change with buttons.
If that is not off, that’s your problem.
I keep that setting off, so the volume buttons don’t mess with VoiceOver.
I just connected a Bluetooth speaker, played a BARD book, playing with the 
volume buttons all over the place, and VoiceOver never got out of line.  I.E. 
Its volume stayed normal.

Steps to reproduce the problem
Add a speaker to Bluetooth settings.
Go into bard and start playing a book
Use the volume up and down button to set the volume of the speaker you’re 
playing through.
While in bard just use voiceover normally.
You’ll find that there are two volumes with voiceover. One of them is the 
volume that you set in the rotor and the other one is actually unknown to me at 
this point that one is very loud and uncontrollable.
 
Questions:
One which volume setting is that voice coming from? To how do you control that 
volume?
Notes
If you want to change set your system up to be normal again just disconnect the 
Bluetooth
