Close all apps, turn off your phone and then turn it on again after 20 or 30 seconds and see if that fixes volume control of Voiceover via the rotor. Apart from that Richard already gave you good advice, the general rule is that the volume buttons control whatever is currently playing. If, for example, SIRI is too loud or not loud enough, ask SIRI something which results in a bit of a longer answer like "What is the forecast for the next week" and then as soon as SIRI starts speaking use the volume up or down button to set it to what is comfortable.
