I'm still on iOS 14.8, and Yes, I am experiencing this issue since an update in mid August.  What I do now is immediately go to the instructions button after adding an item to the cart, then choosing a replacement or to have the item refunded.  This is accessible when shopping, but the screen for doing this after checkout has changed for some reason.  I'm hesitant to update to 15, but as you say, if it corrects the issue I will do the update.

Don't worry, I did remember to complement them on their past efforts at 
accessibility and how helpful the app is, yada yada, so I didn't dump all over 
them, but I wasn't subtle, either.

 I apologize for going back in this thread. What problems are people having 
with Instacart? The problem I am having is everything seems to be in reverse 
order. For example let’s say I ordered mackintosh apples. Instead of it showing 
his mackintosh apples one bag 3 pounds, it is exactly the opposite. Plus, it’s 
not showing what item may be running low. You sort of have to guess based on 
the replacement choices shown. If someone could verify that this is what 
they’re seeing and it iOS 15 make a wreck this problem, I would appreciate it 
very much.
