It probably won't hurt to call them. One last suggestion, do you have a case on your phone? If so remove it and see if that makes a difference, same goes for a screen protector although of course if you remove it you would have to get a new one put on.

While it is off use the opportunity to give your screen a good cleaning by which I mean wipe it down with a damp but not wet to the point where it is dripping cloth and then with a soft dry cloth afterwards. Then turn your phone back on and go back into Voiceover Settings > Audio and turn the Auto Select Speaker option back on and see if that does anything. If that still doesn't help and you don't want to upgrade you can try a Reset Network Settings, it can fix all sorts of issues sometimes. Keep in mind that you will have to know your WiFi password and reconnect to your WiFi after you reboot.
