There are several different volume settings in an iphone. Not all are controlled by the volume setting in Voiceover. So, you may simply have a low volume for phone calls. The way to adjust this is to start a call with someone, have them stay on the line, and then use the volume up button to try to increase the volume of the phone call. Phone call volume is one of the several different volumes in the iphone not controlled by the volume setting on the rotor. I hope this helps.
