That should make the call switch to speaker when you move it away from your head, or if you answered the phone while it was away from your head, if you just cover the upper portion of the screen with your hand and pull it away again, it should flip to speaker. If that setting is on and it is not changing to speaker where the volume will be far better, then there is possibly a problem with the proximity sensor that is by the front facing camera. There are several different volume settings in an iphone. Not all are controlled by the volume setting in Voiceover. So, you may simply have a low volume for phone calls. The way to adjust this is to start a call with someone, have them stay on the line, and then use the volume up button to try to increase the volume of the phone call. Phone call volume is one of the several different volumes in the iphone not controlled by the volume setting on the rotor. I hope this helps.
