That tripped me up too, apparently if you can't make eye contact with the phone and try to set it up without the attention aware feature under accessibility turned off, the phone sets up expecting you to make eye contact with it, probably what people who can see do, but depending on how your eyes are, may be impossible for you.. it is for me, I can look at the phone but not do the eye contact thing.

The phone will give instructions, move head this way and that way, kind of like, when entering a fingerprint one, how it had you keep entering a finger print again and again. Also, in settings, there is a setting inside Accessibility or somewhere, where you turn off Full Attention, although it should be off with Voiceover on.
