I would use Calendar. I would go into the Calendar settings under Settings, Calendar, and set a default alert time for however much time you want to be alerted for appointments. If you don’t want all appointments to sound alerts, then in the details of the appointment you are setting in the calendar, be sure to enable alerts and then you can pick to be alerted at the time of the appointment, 5 minutes before, 15, 30, up to one or two days I think.
Under Sound settings, you want to pick a sound that will get your attention. I find reminders are not so clear even though I have a good sound set. I miss reminders, I never miss a calendar appointment.
