vo on the phone

Hello, this is my first post and I am knew to the iphone!  I have the volume 
up and vo still gets real quiet when the phone rings, I have a hard time 
hering decline or accept.  Is there a solution!  Also, I have some apps that 
won't be silent during calls even after i flipped the switch on the side of 
the phone!  Do I need to turn off vo during calls? Thanks Heather
