My below comments probably aren't worth much but I'll make them anyway. I've had some sort of audio issue with VoiceOver whether it is my selected voice not sounding the same as before the upgrade or something else for the last two or three major iOS upgrades. It normally takes a day or two after the upgrade before my device is speaking properly again.
I have no clue whether this has anything to do with the problem Mary is experiencing or not. I would recommend that she change her default VoiceOver voice to something like Samantha not enhanced and play around with it for a bit to see if her results are any different after doing so. I'm pretty sure I've had to redownload TTS voices following a major iOS upgrade or two in the past to get things speaking right again.
