I have a 4S. Of late, now that I am listening to music on my phone, I am
very unhappy with the volume. iTunes tells me  that my volume is at 100%
when I am copying a CD.   But it isn't loud enough. I am also using a very
old pair of head phones. Perhaps I'd get better results if I buy a newer
pair with volume controls in them?

You need to add volume to your roter first. Go into Settings -> General
-> VoiceOver -> Roter.
