Did you turn on Screen recognition which can completely mess up VoiceOver when it is not really needed. Check under Settings, Accessibility, VoiceOver, VoiceOver Recognition, Screen recognition. Turn it off if it is on and see if that helps. And, if it is in the rotor, make sure to turn it off there.
