I think in that case you'd find your order, open it and there should be
a button showing how many items you have in your order. activating this
should take you to your items. I believe there's a Trash/delete button
next to the item, along with an add button. Probably the quickest way to
do it is to double tap on the item in your order and remove or change
the quantity from within the item's page.
I've found that I've had to use screen recognition more and more with
Instacart and it's getting to be a pain to use. unfortunately, I hear
the web page isn't much better for this kind of thing.

It's doable, but in typical Instacart fashion it's more complicated
than it should be.
I placed an order this morning and realized I'd forgotten something,
so this is how it was for me.
In order to find where to add items I needed to first go to my
account, then orders. I found today's order and double tapped on it.
Once I did this I was taken back into the main screen for the store
I'm ordering from. There was a heading saying the time my order is
scheduled to be delivered. I had to swipe several times to a search
field where I could type in what I was looking for. Instacart popped
up the search results, and I just had to swipe to the item I needed,
then swipe up to the increase quantity on the item I needed and double
tap for that option. A dialog popped up asking which payment I wanted
to use. Once I chose that, the number of items in my order increased
by one.
