Well looks like you identified the problem. It might be a good idea to go into settings and voiceover and I believe it’s under ferocity and voiceover will let you know if the flashlight has been on for a little while. So it’ll actually give you those reminder alerts to let you know when oops moments like this happen. It’s happened to me a few times as well. I sometimes carry my phone in my front shirt pocket and there’s been a couple times that people would tell me that my flashlight was on and I had no clue.
