In VoiceOver Verbostion settings, you can set it to notify you if you leave the flashlight on when you lock the phone. I typed flashlight in the search box at the bottom of settings, and the first item says: Flashlight Notifications , Accessibility → VoiceOver → Verbosity → Flashlight Notifications Make sure that is set to on.
