Hi Tom,
I have the same issue with my sleepwake switch on my iPhone 4s. I know for a 
fact I don't have apple care plan. So, I enabled the assistive touch menu on my 
phone, if that's enabled, I can just go to that menu and there's a button to 
lock your screen on there. If you want more details about the assistive touch 
features on iOs 6, see David Woodbridge's excellent podcast on applevis, I 
think he did it back in October or November some time.  Sorry I can't help with 
your other questions.
