I'd suggest you email the developer. I did that with a question I had once, and 
got a personal reply within hours. Maybe there's a fix the dev can help with, 
if others on this list can't.

I tried this app hoping to get my husband to use it for scheduling his 
medications.  It only talked through the earpiece, and we had no clue what to 
do to make it work.  I will have to find another noisy solution to keep him on 
track and I know there are medication trackers out there, I just have not been 
able to type them all in, would be easy if I could transfer my Word file to the 
phone I suppose but he probably would never read it if I did.  I don't have 
dropbox or any other file-sharing apps at this time.  They can be done, I just 
have not had a great need since all my work stuff comes to me in Word, Excel or 
PDF and I use the computer to access them.  

I know, I am woefully behind the times here, not enough hours in most days.

Thank you for your help.
I'm pleased to report the Alarmed app is just what I was after.
I was able to set a super reminder to occur on the first Wednesday of every 
month.
I love this app to bits!
Don't get me wrong, the default calendar/reminder apps are good, but if you 
want more customization and flexibility, the Alarmed app is the way to go.
It is completely free in the app store, with an in-app purchase for the extras 
pack should you wish to do that. I did this very thing, because I want to 
support the developer with the excellent work he has done to make this app 100% 
accessible with VoiceOver.
Thank you once again, and, as the developer said, are you alarmed? Smile.
