Hi Alan, Yes, you can tap "Decline" when a call comes in, of course as a Voiceover user you would have to double tap it.

Press the Sleep/Wake button twice quickly. 2. Press and hold the center button on your headset for about two seconds. Two low beeps confirm that the call was declined. 3. Tap Decline (if iPhone is awake when the call comes in). I'm not sure if number 3 works with VoiceOver on.
