What you want to do is take a medium size hammer and give the top of the
screen several light taps *smile*. OK, so I hope you don't run off to do
this now before reading the rest of my message.

 

First of all, if you turn Voiceover off with a triple click home and your
phone is in speakerphone mode I assume it would stay that way since there is
no automatic switching if you don't run Voiceover.

 

My first question would be whether you are using a case or do you use your
phone without case?

 

If you are using a case I suggest you take it out and make several phone
calls. Test if the phone continues to switch between speakerphone and
earpiece without any case.

 

If it does and you are sure you hold it definitely near enough to your head
so the proximity sensor should not kick in, then there may very well be a
problem with the sensor. 

 

If everything works fine when the phone is not in a case, the most likely
cause for the problem is your case. Maybe something shifted and the case is
too close to the proximity sensor or is partly covering it. I also was going
to suggest a complete reboot, but it sounds as if you already tried that.
Just to make sure I would remove all apps from the app switcher, then power
off your phone completely. Wait about 20 seconds and then turn it back on.
