When this sensor is blocked, the touch-screen is locked during a call to ensure you do not accidentally trigger something with your face. When VoiceOver is on, pulling the iPhone away from your face causes the phone to switch to speakerphone. This is so you can easily hear VO whilst interacting with the display. For the sighted, there is a speakerphone button on the display. If VO focus is on the call timer during a phone call, the time duration will be automatically announced. The solution is to move VO focus away from the call timer field. Most iPhones leave the focus wherever it was the last time you were on a phone call. Some iPhone 4S owners report VO focus moves back to the call timer on every call. I never experienced this with either my iPhone 4 or iPhone 5.
