VO usually stays active once started. There is only one way which I know of where this is not the case. That way is an occasional bug that hasn't yet been able to reproduce reliably.
If you have a sighted person activate VO for you, go to settings, general, accessibility. The last option will be triple-click home. This option defaults to no action. Double-tap this item and select VoiceOver. From now on, you will be able to turn VO on and off when you triple-click the home button. Turn the iDevice off by holding the power button until VO says slide to turn off, slide or double-tap. Do not hold the power button and the home button for 10 seconds unless nothing else works to reset your iDevice. Though this is Apple's official reboot command, the bug which deactivates VO may occur when this method is used. If so, you will need to use a sighted person or iTunes to reactivate VO. Finally, some people recommend resetting your iPhone by pressing the power button several times rapidly to crash the springboard. This is the only method I have ever used that corrupted data on my iPhone, so I do not use it. This method is not recommended by Apple, and does not do anything unless VO is active on the phone.
