Following on our earlier conversation on assistive touch. That is apparently not going to be an option as VO and assistive touch do not interact well together.

Use either a right single-finger flick to move through the items in the list, or use a four-finger single tap with the majority of your fingers in the lower half of the display to move VO focus to the last item in the list. Accessibility will be amongst the last items. Fortunately, VoiceOver is the first item in the list under accessibility. It says, "VoiceOver on button," when you find it. Double-tapping this button opens the VoiceOver list screen.
