Fred, I understand 2 different resources have been discussed in this thread. One was an NBP publication. The other is an rtf format document that someone offered to forward. I think most traffic has been requesting the document and not the NBP book.
I double checked by going to your provided link and did a search for Robert Miller (the stated author of the rtf document) and got a no results outcome. I could be wrong, of course.
