The major time VoiceOver talking when on a phone call is a problem is when
one has answered that call with headphones, and they can't conveniently get
to their phone to stop VoiceOver. Otherwise, one can just do the two-finger
single tap, and the problem is solved, unless it starts talking again, when
the two-finger single tap stops it, again. Maybe putting the option of
shutting off Voiceover during a call into VoiceOver settings, should apply
only when a call is answered when the screen is locked . Then, if you need
VoiceOver for some reason, unlock the screen and go into the phone section,
or into whatever section is needed, and VoiceOver would function as it now
does.

I could see a problem with VoiceOver automaticly being deactivated during
a
call. What do you do if you need to enter options on your touch tone pad,
such as a bank account number? I want this information to be available to
me. Maybe there could be an option to deactivate all other VO functions
while on the phone, but then you lose the ability to do other things such
as
refer to an email a caller wants to get information from. Oh well,
nothing's perfect.
