Thanks, I was going to ask the same question as Christopher, but closing it in
the app switcher does the trick.
I enjoy Four Down thoroughly. I have often wished I could sit and fill in
crosswords, and now I can. :-)

Unfortunately, the problem that you're seeing is a bug with the VoiceOver
support. It's showing the new puzzle on the screen but is unfortunately still
announcing the clues from the previous puzzle. Please accept my apologies.
The good news is that I've submitted a fix to Apple: I'm just waiting for it
to be reviewed and then it'll be available to you. In the meantime, closing the
app using the app switcher (double-tap the home button to access it) and
opening it again will allow you to tackle another puzzle.
