I too had this behaviar. Turning the phone off and back on fixed it for me. What I did notice is that it only happened after I would place the phone in a speaker dock. I have no idea if that was the cause per say, but just thought I'd mention in case others are using docks and experiencing the same issue after using them. In my case, I'm using a 4S.
