Also, go into settings, general, accessibility, voice over, and then to rotor.  
Add volume to the rotor.  Then, before you start to make one of these call, 
turn the rotor to volume, so that when you are on the call, you can swipe up to 
increase the volume of voice over to a point where you can hear it over the 
automated call voice. I agree that these type calls can be done over the 
speaker, but a problem is that your phone will want to default to the ear 
piece, so you may miss some of what the automated voice is saying until it 
switches back to the speaker.  That is one reason why I prefer to use 
headphones when I know I am going to make one of these type calls.

  VO is quieter, but I can still hear it fine. First, check that your keypad is 
visible, by finding it near the top of your screen or by finding a button to 
show/hide it. If the button is to hide it, the keypad is already visible, 
otherwise double tap to show the keypad. Personally, I use headphones when 
possible if I will need access to the keypad, but I can just as easily use 
speakerphone mode.

keyboard while the phone is making a call? I would be grateful for any advice.

    The page mumbers in Bookshare books are often braille numbers. VDR and 
other apps have their own paging systems, so they will rarely if ever sync with 
what the text claims. Voices are still paid, though you can now use the HQ 
voices offered to VO users in iOS 7 for free.
