There are a few things to consider. First, and most important, the bluetooth earpiece headset, whether mono or stereo, must support the A2DP profile. This profile is normally used for transmission of music over Bluetooth, so almost every stereo headset has it. Nowadays, many mono headsets also support the profile, but there are still many that do not. If you only hear VO in your earpiece during a call, your earpiece does not support A2DP, and the only option is to acquire a new earpiece.
Your second issue is a little more complicated. Bluetooth 2.0, which most earpieces use, has a range of 10 feet. However, when the iPhone is in your pocket, the water content of your body blocks a significant amount of the signal. When I am using my Bluetooth "b.0 headset, I cannot get a proper connection if the phone is in the pocket on the other side of my body. If I use a coat pocket, the phone is usually far enough away from my body that the signal is not strongly mitigated by my body. In other words, I can use the phone across my body, unless the jacket construction has materials that dampen the signal. That said, I have a Bluetooth headset which uses Bluetooth 3.0. This profile permits a signal of up to 75 feet line of sight. When the iPhone is in my pocket on the opposite side of my body, I get a clear signal most of the time. There can still be some signal degradation occasionally, but it usually works. However, if I put my iPhone in the pocket on the same side as my earpiece, it works quite well. The Bluetooth 3.0 earpiece I use is the Motorola H19TXT. Note: the H19 does not provide A2DP profile. It must be the H19TXT model.
