You can't do anything about VoiceOver repeating the word delete after you delete a message. I'm guessing that the delete button is getting focus again once the new message is loaded. I just wait to hear delete and then I do a two finger swipe up to read the message.

If you're reading a message then the number of unread messages is listed next to the In Box button at the top of the screen that takes you back into your In Box.

I'm not sure I've noticed your last point. I hear the word message and then it starts reading the message. I suppose if you have a blank line at the beginning of the message you're trying to read, you may not hear anything. You also may have to wait a beat before it starts reading the message.
