Also in Outlook, make sure your contacts are stored in the iCloud data file rather than a local data set. Press CTRL+Y to bring up the folder list, and make sure they are stored under the "iCloud" section.
Also check under the Add-ins section in the options dialog to make sure the iCloud add-in hasn't been disabled. Outlook occasionally disables this on its own accord if it thinks that the add-in is slowing down Outlook too much.
