Just to add to this, I simply lay the lens of the camera right on top of the code (the code is the smooth part of the card, under the peel-off piece of plastic; it is a long, narrow rectangle). Once positioned, and once you are sure you have a good light on in the room, just lift the phone straight up. Once the code is far enough away to fit, the phone will automatically capture it, no need to manually take a picture or anything. While I don't know if it matters or not, I always have the alignment such that the bulk of the card is facing down. That is, the code is parallel with the lock button, and the card extends down toward the charging port while the code is up near the camera. I hope that makes sense.
