The app I am thinking of is developed by MobGem. I believe that is how it is spelled. What you do is play the sound, then you have to press the download or close button to get back to the list of songs and it can be a little cumbersome to do. They do have some neat sounds.

If so it appears that it's just another app like Ringtone Maker, Ringtone Wizard or Ringtone Architect. All of these apps basically allow you to pick a song from your music library, select a portion of that song and convert it to a ringtone. You then have to either email this ringtone to yourself or get it out of the app using iTunes file sharing, then save it to your ringtones folder, import it to your tones library in iTunes and sync it back to your phone. There simply is no way to get ringtones to a non-jailbroken iPhone without using iTunes. The only exception is if you buy the ringtone from iTunes, then it's downloaded to your phone and made available. I also heard that if you use Apple's Garage Band app you can make a ringtone and somehow make it available on your phone and all of it from the app without having to sync through iTunes. I have not tried this so it's just hear say.
