So may sound dumb here but on my phone (iPhone 4S) I am having inconsistencies with always speak notifications,
Caller id will be spoken most of the time when this setting is turned on however if I miss a call voiceover sometimes will say who the call is from that is displayed on the lock screen other times it will not… same with text messages -- The following information is important for all members of the viphone list.
