I have unlocked my fair share of phones, in my time, and, as far as I know,
the only way to confirm that your phone is truly unlocked is to insert a SIM
card of a different carrier from that from which it was unlocked.  

Now I haven't unlocked a phone in several years but way back then, I used to
go into a store of a different carrier and ask one of the sales people to
insert one of their cards to see if it would work.  

Perhaps there are apps for this sort of thing, these days, but none of which
I am aware.

Recently I requested an unlock code for my iPhone 5 which I am selling to
Gazelle.  I got confirmation for my phone to be unlocked from AT&T and given
instructions to backup and restore my phone through iTunes which I did.  I
didn't get any message stating that my phone was unlocked.  Is there a way
to tell if your phone is truly unlocked?
