Can't understand the case connection, but know there is a setting under 
Sounds so that you use a single volume command, though think it stops rings 
and things being set independently, but know it has a bearing.

Also,  if you get VoiceOver to keep talking, like do a 2 finger flick up so 
it has to jabber down the entire screen,  then crank the volume buttons 
while it does it.

Worked for a good many so far.

I have the 5s. I am not sure the IOS version is relevant to my question. At 
the time of incident  I was running 7. Every time I put the phone in the 
case it would turn the volume way down and I could not turn it up until I 
took it out of the case. I could get seri to come back up but not the voice 
over. I then had to get sighted assistance to increase the volume of the 
voice over. Has anyone else had this problem?
