Can't understand the case connection, but know there is a setting under Sounds so that you use a single volume command, though think it stops rings and things being set independently, but know it has a bearing.
Also, if you get VoiceOver to keep talking, like do a 2 finger flick up so it has to jabber down the entire screen, then crank the volume buttons while it does it. Worked for a good many so far.
