I got my phone completely replaced about 8 weeks ago after the battery replacement program was announced. I had the battery issue as well as the lock button one so the Apple guy just swapped my device out for a brand new one (I still had Apple Care).

There was some confusion when I brought the phone to the apple store because somewhere on their page it's indicated that phones need to have been purchased less than 2 years before but I got mine accepted. Later I double-checked with apple on the phone and they say that every phone exhibiting the lock button problem with eligible serial number will be accepted for repair. If anyone has payed for it or for a phone replacement because of this issue it's worth contacting apple. You can send the phone in by mail but I'm not sure if they would give you a loaner and that's why I chose to go to the store. Incidentally the nice thing is that they'll also replace the battery while they're at it.
