I've tested with the example that you've sent to me by e-mail (שדגכ.doc), and the filename works fine here, under Linux. (Not sure about the right-to-left writing, but probably that's another story.)
In vm-mime.el I don't see anything specific to filename coding, the name is passed unchanged to write-region for saving, or for writing of a temporary file.
