Even better, I think I have the cause... IdleTimeout was set to 0 in the
registry.

Now the question is why?  I'm 99% sure that I've never messed with the
IdleTimeout.  All the machines I have problems with (I did have it work on
some after more testing) had been upgraded from 3.3.7.  The also have
IdleTimeout set to 0 in the ORL keys.  Could there be an extra "Import 3.3
Settings" happening in the install?
