This setting is largely a matter of personal preference and the blame honestly lies in the zillions of mail clients for whom 'reply' is the default instead of 'reply-all'. Plain 'reply' is almost never what you want, I recommend learning the hotkey for reply-all ... this problem can't just be biting you in mailing lists.
alex P.S. Incidentally, the hotkey for reply all in gmail is 'a'. Be sure to turn hotkeys on in your preferences if you use the gmail web interface, and maybe consider getting the greasemonkey script for additional shortcuts. In gmail, if you forget to reply all, you can click it after the fact while you're still composing your message; it appears just above the send and save buttons.
