Ports need to be properly forwarded as well as being opened.  Look here:

Hi,

I am using RealVNC 4.1.2 free edition to connect between two XP Pro boxes.

Connecting from within my organisations network (from my work computer to
the box running VNC Server) is no probs, but when I VPN in to the network
from home I get no password dialog when attempting to connect.  Using either
the program or the java viewer, after putting in the computers static IP,
the connection details dialog box disappears but no password dialog appears.

All firewalls are set up to allow port 5900 (which is the port I am using to
connect with).  A port scan from my home computer when VNC'ing in shows that
port 5900 is open.

Can someone shed some light on my little issue?
