Proceeding from the assumption that voice is a public utility and has regulatory involvement (e.g. public safety, civil emergencies), and not World of Warcraft, leads to the conclusion that some sort of systematic, integrated admission, egress and routing policy control is needed.

There hasn't been anything like industry-wide agreement as to what those things should be. Eventually someone like Inteliquent shows up and decides for you, with a view to their bottom line, of course.
