Yes, the installer is run from the terminal. The installer is an ncurses-like text interface. It doesn't make use of a GUI, so even if Orca came on the ISO, it could not be used with the installer. (Unless i'm misunderstanding Orca's capabilities.)

If you need Orca for the install process in particular, then the preceding applies. If you don't necessarily need Orca for the install process, but need it more generally, then even though it doesn't come with the ISO, you could still easily install it after installation by running:

   # xbps-install gnome-orca

If you want to create your own live ISO containing MATE and Orca, follow the instructions at:

   https://github.com/void-linux/void-mklive

That's probably also the place to request that Orca be made available on the live ISOs by default.
