But...my understanding of the proposed solution is that they can't go anywhere other than your page once you change the DNS server. So in effect they are in fact cut off at that point. Am I missing something?

We have no problem cutting people off for non-payment regardless of their service level. In fact, I had to disable a T1 customer today. Needless to say, they called within the hour to arrange payment.
