"Hours later." Surely that tells you heat was being generated.

"No, it was just debris or something. This sort of thing happens with experiments."

It would require BOTH the inlet and outlet to be blocked. The inlet was probably a peristaltic pump with plastic tubing that wouldn't take that much pressure without it being obvious.
