I've been reading some articles on Hooking up with steelhead, here are a few things 
that might cause for more hookups.  

1.  Filed point for easier penetration
2.  A different hook.
3.  When a fish hitts, pull rod to side or right at you.  rather than straight up.   
Straight up on a swimming fly I guess tends to not allow it to get into the mouth very 
well.

I'm no expert, Just relaying some info I have read.
