1/ Preview does not show all types of annotations or interactive
elements in PDFs, so if the column headings were encoded as
interactive annotations, they might not show up in Preview.

2/ To explore this further: If you try to select the text in the
document, including the area where the headings should be, do the
headings appear in any way (e.g. are they white-on-white or
something)? Also, if you use Preview to 'search' for the titles, can
it find them? If the answer is "no" to both of these questions,
perhaps the headings are annotations. Otherwise, I do not know why,
but perhaps there is a new version of PDF that Preview doesn't render
correctly.
