Web Designer: Artsy kinda person who's educated in design and knows
Photoshop (or something similar) and HTML.

Web Content Developer: Depends on where you work, but usually means
someone who's responsible for the meat of the site.  It could be text,
articles, reviews, adds, product info.... What ever the given site is
about.  Often more of an editor type position.

Sure, someone could do both.  The smaller the company the more people
will wear multiple hats.  Independent contractors will often do it all.
